If you would like to request a copy of your records, please request the Records Release form, and fax it to us at our main fax number listed on the form.

Doylestown Health Gastroenterology accepts the same insurance plans as Doylestown Hospital.

Please bring your current and correct insurance card at the time of your visit.

HMO and other "managed care" plans require that your primary care physician provide you with an electronic referral to the specialty care physician. Please contact your Primary Care Physician at least (7) days in advance of your visit for this referral to be processed. If your insurance provider requires a referral and your Primary Care Physician has not yet generated one to our office, we may reschedule your visit.

Patients are responsible for full payment of any co-payments, co-insurance, and deductibles required by their insurance company at the time of your visit.
