New Patients

You will be asked to complete a New Patient information form provided to you at the time of your first visit to Doylestown Health Gastroenterology.

Open Access Colonoscopy

Doylestown Health Gastroenterology's Open Access program allows healthy patients without exclusion criteria (see enrollment form for exclusions) to schedule a screening colonoscopy without an initial office visit.

Medical Records Release

If you would like to request a copy of your records, please request the Records Release form, and fax it to us at our main fax number listed on the form.

Insurance and Billing

Please bring your current and correct insurance card at the time of your visit.

HMO and other “managed care” plans require that your primary care physician provide you with an electronic referral to the specialty care physician. Please contact your Primary Care Physician at least (7) days in advance of your visit for this referral to be processed. If your insurance provider requires a referral and your Primary Care Physician has not yet generated one to our office, we may reschedule your visit.

Patients are responsible for full payment of any co-payments, co-insurance, and deductibles required by their insurance company at the time of your visit.

Payment Options

The patient is financially responsible for any deductibles, co-insurance, and non-covered services. We accept Visa, MasterCard, and Discover.
