Make your visit to Doylestown Health Urgent Care as smooth as possible using this checklist. Be on the mend quickly. Walk-ins welcome, or check-in by phone or online to let us know when to expect your arrival.

      We want to make your Doylestown Health Urgent Care visit as quick, convenient, and satisfying as possible. Whether you check-in in advance by phone or online or simply walk-in; this checklist should help to expedite your trip.

      Please bring these items with you to speed the process:

      • Your valid current insurance card. We accept these insurance plans.
      • A current driver's license with photo identification or a current government-issued ID card.
      • A list of current medications, including dosage/strength, as well as all over-the-counter medicine, vitamins and supplements.
      • Your self-pay or co-pay fee (the amount varies based on the insurance plan). We accept cash, checks and credit/debit cards (except American Express).
      • Completed Patient Registration form and HIPAA acknowledgement form
