How to Request a Copy of Your Hospital or Emergency Records

Doylestown Health now offers an online records request tool that verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

You may also obtain copies of medical records. Please contact Health Information Services at 215.345.2314 or download the authorization form (pdf). Copies of records are available on CD upon request.

Hours of operation are Monday through Friday from 8:30 am to 5 pm.

Voicemail is available to leave messages. Please provide 48 hours notice to prepare your records if necessary; however, in an emergency situation, we will do our best to expedite a request.

To ensure patient confidentiality, federal regulations (HIPAA) require photo ID and signature of the patient at the time of pick up. If you are unable to pick up your records, please call to make other arrangements.

The Health Information Services (HIS) Department is located on the ground floor of Doylestown Hospital. Take the elevator from the Main Lobby to the ground floor.
E-Notification Request

Doylestown Hospital has chosen PatientPing to facilitate the delivery of real-time event notifications for care events occurring at its facilities, as required by CMS’ electronic event notifications Condition of Participation (e-notifications CoP)*.
